- Team member names, including your own name. (You rate yourself, too!)
- Percentages
- Percentage of team marks that this member deserves. (Enter 100% for a team member who participated fully in the project.)
- Reasons
- If Percentage is less than 100%, enter the reasons why you rated this person as failing to contribute 100% to the team effort. (e.g.: Failed to show up for meetings; failed to carry out agreed tasks; poor quality work; lack of cooperation, etc.)
